The New Records System For Birth Parents was developed to provide an opportunity for birth parents of an adopted person to indicate a preference regarding contact with the adopted person, subsequently requiring submission of family history information which will be released to the adopted person and certain other individuals along with the adopted person's original birth certificate upon request.

Click on Download forms to print and mail contact preference, family history, and / or redaction request forms to the NJ State Registrar. If you choose this option, you should not submit the forms through this system.
